Our commitment to your health information
Dental Illusions is required by law to maintain the privacy of your protected health information (PHI), to provide you this notice of our legal duties and privacy practices, and to follow the notice currently in effect.
How we may use and disclose your health information
For treatment — providing your dental care and coordinating it with other providers involved in your care. For payment — billing you, your insurance plan, or others on your behalf, including verifying benefits. For health care operations — running the practice: quality review, training, scheduling, and appointment reminders by phone, text or email. Other uses and disclosures — as required by law, for public health and safety, and in other limited circumstances permitted by HIPAA. Uses beyond these — including most marketing uses and any sale of your information — happen only with your written authorization, which you may revoke at any time.
Your rights
You have the right to: inspect and receive a copy of your dental records; request corrections to them; request restrictions on certain uses and disclosures; request confidential communications (for example, a preferred phone number); receive an accounting of certain disclosures; receive a paper copy of this notice on request; and choose someone to act for you. To exercise any of these rights, contact the practice at (713) 715-7354 or in writing at the address above.
If you believe your rights have been violated
You may file a complaint with the practice using the contact details above, or with the U.S. Department of Health and Human Services Office for Civil Rights. We will never retaliate against you for filing a complaint.
